ACE-inhibitor–induced angioedema occurs due to bradykinin accumulation, as ACE normally degrades bradykinin. This leading to increased vascular permeability and sudden facial and/or airway swelling. Risk factors include prior angioedema, female sex, older age, smoking, chronic kidney disease, and concomitant use of painkillers. Management requires immediate discontinuation of the ACE inhibitor, careful airway monitoring, and supportive treatment. Antihistamines and steroids have limited benefit.
